If you have concerns or require any information on insurance claims, please discuss this prior to your appointment by phoning the office.
No Show
In the event that you will not be able to keep an appointment, you must notify the office 48 hours in advance of your scheduled appointment. If the office does not receive an advance notice and there is no clear emergency, a charge of $40.00 will be posted to your account.
